Model Privacy Notice (1)
1．Contents of Information
The following items of information should be provided concerning the handling of personal data.
1) Outline of the service to be provided
2) Personal data to be acquired and method used for its acquisition
3) Purpose of using the personal data
4) Whether or not personal data and/or data obtained by processing personal data will be
provided to third party and, if so, to whom
5) Whether or not the consumer can suspend/correct the provision of personal data and, if
so, how to do so
6) Point of contact for inquiries
7) Period for which data are stored and disposal of data
2．Explanations to be Provided
1) Methods of description concerning the personal data to be acquired, and method used for its
acquisition
- The items of personal data to be acquired and the method used for their acquisition should
be broken down into as detailed categories as possible and described in concrete terms.
- Those items of personal data to be acquired and the method used for their acquisition that
are difficult for consumers to understand should be described clearly.

2．Explanations to be Provided (cont.)
2) Methods of description concerning the purpose of using the personal data
- The purpose of using the personal data to be acquired should be specified and described
in concrete terms.
- The purpose of using the personal data should be described in relation to each item of
personal data to be acquired.
- Those purposes of using the personal data to be acquired that are difficult for consumers
to understand should be described clearly.
3) Methods of description concerning whether or not personal data and/or data obtained by
processing personal data will be provided to a third party and, if so, to whom it will be
provided
- If the personal data to be acquired by the business operator or data obtained by
processing personal data is to be provided to a third party, the party to which it will be
provided (including the conditions for selecting the party to which it is provided, if said
party will be changed after the fact) and the purpose of providing it should be described.
- If data obtained by processing the personal data acquired by the business operator is to
be provided to a third party, the method of processing it should be described.
4) Methods of description concerning whether or not the consumer can suspend the provision
of personal data and, if so, how to do so
- The description should states whether or not it is possible for consumers to halt acquisition
or suspend the use of personal data by business operators and, if so, discloses how to halt
its acquisition or suspend its use.
