ATTENDANCE POLICY APPEAL FORM
2014-2015
This form is FOR:
Students who have exceeded ten (10) absences in a block
course and have extenuating circumstances on which to
appeal the Attendance Policy. Students appealing the
policy must have a passing average in the course.
This form and all other supporting documents must be
submitted by January 9, 2015.
LATE APPEALS WILL NOT BE ACCEPTED!!!
If you wish to appeal the JT Hoggard High Attendance Policy, please do the following:
1. Complete this form.
2. Attach a statement from the student explaining the absences and the rationale for waiver (See Question 3).
3. Attach a statement from the parent/guardian supporting the waiver (See Question 4).
4. Attach all supporting documentation.
5. Return this form and all documentation to Mr. Newton’s office by January 9, 2015, at 3:30 P.M.
Your attendance records, disciplinary records, and teacher recommendations will be considered in making a decision on
your appeal.

3. Please explain the extenuating circumstances you feel should result in a waiver of the JT Hoggard High School
policy allowing you to receive the grade you earned in spite of your excessive absences. You should attach this
explanation to your “Attendance Policy Appeals Form.” Please ensure that your explanation focuses on specific
circumstances regarding the student filing the appeal and not an appeal of the policy itself. Attach with this all
medical or legal documentation that is not already on file in the Attendance Office.
4. Attach a statement from your parent or guardian supporting your appeal request.
5. What is the name of the staff member that we should contact that you have been working with this semester
regarding your attendance issues?
All questions (1-5) MUST be addressed for appeal consideration.
All forms and documents are due on January 9, 2015.
