Father’s name and Mother’s name. 4. Full residential and office postal addresses and telephone number(s). 5. Signature of Parents/parent. 6. Self-addressed (9.5” x 4.5”) envelope, NOT SMALLER. Opening date : Monday, 2nd February 2015 Closing date : Monday, 16th February 2015 Holidays : Saturdays, Sundays and General Holidays Working hours : 9 a.m. to 3 p.m. IMPORTANT: The Admission Form must be filled in and returned to the School Office on or before Friday 27th February 2015.
