Services are deemed to be rendered in the State of Texas when: For Independent Clinical Laboratory (Lab): The referring provider is located in the State of Texas. For Durable/Home Medical Equipment and Supplies (DME): The equipment is purchased at a retail store in Texas, or the equipment is shipped to Texas. For Specialty Pharmacy (for non-routine, biological therapeutics that generally includes injectable and infusion therapies, high-cost therapies, and therapies that require complex care): The ordering physician is located in Texas.
